ENROLLMENT REQUIREMENTS

If you are an incoming 6th grade student previously enrolled in one of Covington Independent District elementary schools, we need updated health records, including a physical and updated shot records, and a current census packet must be completed in order to receive a schedule.

NEW TO THE DISTRICT?

Required documents needed to enroll at Holmes Middle School:

• Proof of Residence (utility bill, mortgage, lease/rental agreement) 
• Birth Certificate and/or Custody Papers
• Social Security Card or Statement of Non-Disclosure of SS Number
• Immunization Records (due upon entry on a KY form. Forms available at Health Care Provider or Health Dept)
• Physical Examination
• Last Year's Final Report Card
• Special Education Records (if Applicable)
