Immunization records due by May 24

Don’t forget to get your shots and turn in your immunization records to your school by May 24.
All students in kindergarten through grade twelve are now required to have two doses of Hepatitis A vaccine. Additionally, all students age 16 years or older must have two doses of Meningococcal ACWY. These are new requirements in Kentucky's law.

To further satisfy these requirements, a current Certificate of Immunization Status from your child's healthcare provider must be turned in to the school. Your child may have already received these vaccines. If so, all you need to do is turn in the current immunization certificate to your school nurse by May 24.
 

If your children still need these vaccines, they can get the immunizations from their doctor, from the local health department or through our school partnership with HealthPoint. In order to get the vaccine at school, your child must be registered with the School-Based Health Center provided by HealthPoint. Contact your school's nurse with any questions.
